Primary Care Nurse

You will be assigned a primary care nurse who will coordinate the care with other members of the interdisciplinary group and handle any nursing needs. The role of the nurse is to help prevent and relieve pain and other symptoms, assist in monitoring medications, and to teach caregivers ways to provide for the patient’s care.

Meridian Hospice Care, Inc. uses nursing care to help patients manage symptoms, maintain comfort, and receive appropriate clinical support throughout their hospice journey. Nursing services are part of the Medicare hospice benefit when they are included in the patient’s individualized plan of care.

How Hospice Nursing Services Work

The hospice nurse evaluates the patient’s current condition and works with the interdisciplinary team, patient, family, and attending physician to address changing needs.

  • A hospice nurse assesses symptoms, comfort needs, medications, and changes in the patient’s condition.
